Google Forms

My name is Beth Still and I am a high school social studies teacher in Nebraska. I have used Google Forms for a few years in my classroom. I would like to take this opportunity to share a few things about Forms that will hopefully inspire you to learn more about how you can get the most out of this terrific app!


Google Forms is a tool that allows you to collect information quickly and efficiently. A Google Form can be connected to a spreadsheet which makes it even easier to analyze information. Many teachers have found Google Forms to be extremely useful in their classrooms. They use them for a variety of purposes from collecting a lunch count each day to surveying parents about what new books they would like to see purchased for the school library.

To access Google Forms you will need to log into Google Docs. From there you will need to click Create then select Form. You will immediately be prompted to give your form a title and select a theme for it. Themes dress up your form and make them visually appealing. There are a variety of themes so chances are there is one that will fit your needs.

When you set up your form you will need to decide how you will accept responses. You can either choose to create a new spreadsheet or you can keep the responses only in Forms. I recommend creating a new spreadsheet each time because it allows you to see the information you collect more easily.

After you have added a title, selected a theme, and decided where you will collect responses, it is time to start adding information to your form. If desired, you can include a description. A description helps your audience have a better understanding of the purpose of your form. The next step is to begin adding questions. In addition to adding questions you can also add images and videos. You can also add more pages and design questions so that responders are taken to specific pages depending on how they answer each question.

One-line text box to enter response. This type of question is good for very short answer questions.

Paragraph Text

Multi-line text box which lends itself well to open-ended essay question.

Multiple Choice

This type of allows you to enter as many options as you wish. You can also copy and paste a list into Option 1 which will automatically separate the choices into different options. This this type of question allows users to select one of the choices.

Checkboxes

Once you type in your question you can enter as many options as you wish. Users can select as many options as they wish.

Choose From a List

This type of question allows you to enter as many options as you want to. Options will appear on a drop-down list. Users may only select one choice.

Community_Identity_Voting

Scale

There are a couple of options for this type of question. Once you enter your question you will set up the scale which will have to start at either 0 or 1 and go as high as 10. You can opt to use words such as “Agree the least” and “Agree the most.” Responders will click the bubble that indicates their preference.

Grid

This type of question allows you to add multiple questions and have a scale that applies to each question. Grids are very much like scale questions, but there are multiple questions instead of just one.

Date and Time

With these types of questions the user is asked to enter a date or time of day.

Before you are ready to send your form out, you will have a few more decisions  to make. Once you have created all the questions you will be directed to a page where you can decide if you want to show responders the link to the form again so they can submit another response. You will also be given the option to publish a link to the results that all respondents can click to see all the responses. Finally, you will be able to decide if you want responders to have the ability to edit their responses after submitting.

Once you have completed these steps you are ready to send your form out. You can do this in many ways. You can send it out as a link using Google+, Facebook, or Twitter. You can enter the email address of the recipients as well. If you are sending the link to someone on paper you can use a URL shortener such as goo.gl or bit.ly to create a short URL which is easier to work with. If you have a blog or website you can capture the embed code and you can embed it. One of these options will probably work for you.
